Welcome to King School!
All enrollment is now on-line. You will want to start from the district website at cypsd.org, under “parent” tab and then enrollment. Before you start, please save to the device that you will use the following documents. All supporting documents will need to be uploaded into your on-line enrollment packet.
The documents that we need are:
1. Birth Certificate – please scan and upload into your enrollment packet
2. Residency Affidavit (attached) – please complete & sign and then scan and upload into your enrollment packet
3. Driver’s license – please scan and upload into your enrollment packet
a. If your driver’s license does not have the correct address on it, you will also need to change your address with the DMV and upload one of the following as well:
i. Brown card if you change your address in the DMV office
ii. Screenshot if you change your address on the DMV website
4. Utility bill (gas, water, electric) or rental agreement – please scan and upload into your enrollment packet
5. Heath History form (attached) – please complete & sign and then scan and upload into your enrollment packet
6. Immunization card – please scan & upload into your enrollment packet (please make sure that your child’s name is on both sides of the yellow card)
7. Kindergarten Physical (attached) – please scan and upload after the doctor has completed and signed
a. If this is not yet completed, please make an appointment and email me the date of the appointment. We will process the enrollment while waiting for the form
8. Dental exam – please scan and upload after the dentist has completed and signed
a. If this is not yet completed, please make an appointment and email me the date of the appointment. We will process the enrollment while waiting for the form
Once all of the above documents have been uploaded into your enrollment packet, please use the “finish and submit” button at the very end to send the packet back to me to finalize.
